IS HOSPICE THE ANSWER?

HOPE is committed to the delivery of high quality, compassionate patient and family focused care. This is the cornerstone of our organization’s Open Access philosophy, which ensures that HOPE will accept any person eligible for Hospice Care regardless of the complexity of their medical needs. Anyone may request an information visit or make a referral to HOPE 24 hours a day - 7 days a week – 365 days a year. Have you or your loved one…..

  • been hospitalized or gone to the emergency room several times in the past year?

  • needed more frequent contact with your personal physician over the past year?

  • started taking medication to lessen physical pain?

  • been less active – spending more time in a chair or bed due to fatigue?

  • fallen several times over the past year?

  • began to require help from others with personal care such as bathing, dressing, walking, eating and cooking, or getting out of bed?

  • started to feel weaker or more tired lately?

  • experienced unintentional weight loss?

  • been short of breath even while resting?

  • been told by your doctor that your life expectancy is limited?
